Some insurance plans require a physician referral for nutrition counseling. If yours does, you can upload it directly through your Fay account. Once uploaded, your dietitian and our team will review it, and we'll reach out if we need anything else!
Accepted file types: images, PDFs, and Word files up to 10 MB.

Tap Documents
Tap + Add a document
Select your referral file from your device
Your document will appear under Documents once uploaded

Click Settings in the left sidebar
Click the Documents tab
