Some insurance plans require a referral or documentation of a qualifying medical condition for nutrition counseling to be covered. If your plan requires this information, you can upload it directly through your Fay account.
What should I upload?
Please upload one of the following documents from your medical provider:
A physician referral that clearly documents your qualifying medical condition
A note from a recent visit that clearly documents your qualifying medical condition and is signed by your medical provider
Make sure the document clearly includes:
Your qualifying medical condition
Your provider’s name
Your provider’s signature
The date the document was signed
You can use this sample referral template to make sure your documentation includes the necessary information. Once uploaded, the Fay team will review your documentation and reach out if we need anything else.
Accepted file types: images, PDFs, and Word files up to 10 MB.

Your document will appear under Documents once uploaded

What happens after I upload my documentation?
The Fay team will review your documentation to determine whether it meets your insurance plan’s coverage requirements. If you meet your plan’s coverage requirements and submit the required documentation, Fay will work to rebill eligible appointments that were denied coverage or had cost sharing applied.
You may be responsible for the cost of an appointment while we wait for your insurance plan to process the updated claim. If your insurance ultimately covers an appointment you already paid for, Fay will refund the applicable amount. Our goal is to get eligible appointments covered and rebilled wherever possible.
