Why do I need a referral for an HMO plan?

With an HMO (Health Maintenance Organization) plan, your primary care provider (PCP) manages your care. Many HMOs require a referral from your PCP for specialty services, including nutrition counseling.

Submitting a referral gives your claims the best chance of being covered, though coverage is ultimately your plan's decision.

How to submit a referral:

  1. Confirm with your PCP that your HMO plan allows you to receive services from providers outside of their network.

  2. If yes, ask your PCP for a referral for nutrition counseling. If you need a sample referral form, you can find it here.

  3. Upload your referral by following these steps.

  4. Our team will review it and reach out if anything else is needed.

To confirm what your plan requires, contact your insurance company using the number on the back of your card.
