Possibly. Some insurance plans cover nutrition counseling as a preventive benefit, which may result in lower or no cost to you when certain plan requirements are met. If the claim or supporting documentation does not reflect a qualifying health condition, your plan may process the visit under your medical benefit instead. This is where copays, deductibles, and coinsurance may apply.
Step 1: Call your insurance company
To confirm your plan’s requirements and get the most accurate information about your specific coverage, we recommend contacting your insurance company using the number on the back of your insurance card. Have your member ID and date of birth ready. You may also need Fay’s NPI: 1104334465.
Ask them:
Which benefit was applied to my Fay appointment: preventive or medical?
Does my plan cover nutrition counseling as a preventive benefit for certain health conditions?
If so, which conditions qualify?
What diagnosis information or documentation is required for the visit to be covered as preventive?
Write down the date and reference number of your call in case you need to follow up.
Step 2: Send us your documentation
If your plan confirms that you may qualify for preventive coverage, upload documentation showing the qualifying condition to the Docs & Forms section of your Fay account. This may include:
Medical records showing the qualifying condition
A signed note from your physician documenting the condition
A referral from your doctor or an approved referral from your plan that documents the condition. If you need a sample referral form, you can find one here.
Follow these steps to upload your documentation.
What happens next
Our team will review the documentation you submit and resubmit any eligible claims to your insurance company. If your plan reprocesses a claim with lower or no cost to you, we’ll refund any amount you paid above your updated responsibility. Final coverage decisions are made by your insurance plan.
